Clinical Rules for Pediatric Dextrose Infusion:
- Peripheral IV Route: Maximum safe dextrose concentration is 12.5%. Concentrations > 12.5% mandate Central Venous Access (e.g. UVC, PICC, Central Line) to prevent severe chemical thrombophlebitis and extravasation necrosis.
- Fluid Selection Flexibility: Mix any available lower stock (D0, D5, D10, D15, D20, D25) with a higher stock (D10, D15, D20, D25, D50). The calculator automatically picks the closest bounding fluids and computes the exact volume ratio.
- Blood Glucose Monitoring: Re-check capillary blood glucose (GRBS) every 30–60 minutes during active fluid titration until euglycemia (45–120 mg/dL in neonates, 70–140 mg/dL in children) is established.
